Supporting Carers for Better Health and Wellbeing

At Wembley Park Medical Centre, we understand the vital role carers play in supporting their loved ones. That’s why we’ve developed a comprehensive approach to identify and support carers, ensuring their health and wellbeing are prioritised alongside their caregiving responsibilities.

If you are a carer or know someone who is, please let us know. We have a dedicated carer support service facilitated by our social prescribers and health and wellbeing coaches who can assist you in your caregiving journey.

Our Commitment to Carers includes creating a supportive environment, promoting positive wellbeing, preventing crisis and family breakdown and addressing inequalities. We have appointed Dr. Ishani Patel as a strategic lead for carers and our Carers Lead/Champion. Our practice has dedicated Social Prescribers and a Health and Wellbeing coach who provide health and wellbeing assessments tailored to carers’ needs, with flexible scheduling options to accommodate their caregiving responsibilities. We provide comprehensive information and signposting both within our practice and through partnerships with local carer organisations, social services, and voluntary sector. Our staff undergo regular carers awareness training to better understand and support the needs of carers within our community. Together, we can ensure that carers receive the recognition, support, and care they deserve.

We work closely with a range of health and social care providers, including Family Action, Carers UK, Brent Carers Centre, and others, to ensure seamless support for carers.

Brent Carers’ Centre is an independent Charity and Company Limited by Guarantee. Carers Centre’s have been supporting unpaid adult and young carers since 1994. Their vision is “To ensure that unpaid carers are recognised, valued and supported to live rather than just exist. Brent Carers Centre deliver a holistic range of support services to address the social, emotional and financial issues that carers experience.

For young carers, the Just Talk Service provides confidential space for young carers to talk about how they are feeling and provides support, tips and solutions to help address or manage circumstances. Experienced counsellors are available over the phone or in person.

We also recommend carers to have their annual influenza vaccination
